Step 2: Within 5 working days, the Department of Food Safety will check the file. If the dossier is invalid, the dossier shall be supplemented and the dossier completed.

Step 3: For valid dossiers, within 10 working days, the Department shall decide on the establishment of appraisal teams and organize appraisal in the field. Eligible establishments will be issued certificates of eligibility for food safety. Step 4: Return the certificate of eligibility for food safety to the facility.

1. An application for a food safety eligibility certificate (made according to a set form).

2. Business registration certificate with registered food business (copy certified by the establishment).

3. Explanation of material foundations, equipment and tools to ensure food safety conditions (with certification by establishments), including:

a) A drawing of the plan of the ground design of the establishment and the surrounding area;

b) The diagram of the food production process or the process of preservation and distribution of products and explanations on the material foundations, equipment and facilities of the establishments.

4. A list of food safety knowledge certified by the competent authority

5. The list of health examinations certified by the owner
