APPLICATION FOR GRANTING CERTIFICATE OF BASIS OF FUNDING FOOD SAFETY

1. Dossiers of application for certificates of eligibility for food safety.

A dossier of application for a certificate shall be made in 1 copy, comprising the following papers:

a) An application for the certificate of food hygiene and safety standard;

b / The business registration certificate which is registered for food business (copy certified by the establishment).

c / Explanations on material foundations, equipment and instruments meeting the food safety conditions (with certification by the establishments), including:

– Drawings of the ground plan of the establishment and the surrounding area;

– A diagram of the food production process or the process of preservation and distribution of the product and a description of the facilities, equipment and facilities of the establishment.

d / A certificate of training of food hygiene and safety by the establishment owner and the person directly engaged in food production and trading:

– For establishments with less than 30 persons: Submit copies of certificates (with certification of establishments);

– For establishments with 30 persons or more: Submit the list of trained (certified by the establishment).

e) The health certificate of the establishment owner and of the person directly engaged in food production and trading; Report on the results of negative culture of pathogens causing intestinal diseases (cholera, dysentery and typhoid) of persons directly producing food in areas where exist diarrheal diseases according to the announcement of the Ministry of Health :

– For establishments with less than 30 persons: Submit copies of certificates (certified by establishments);

– For establishments with 30 persons or more: Submit the list of results of health examination and test of faeces of establishments’ owners and participants directly engaged in food production and trading (certified by establishments).

2. Order of execution.

The procedures for granting certificates are as follows:

a) Examination of dossiers:

– Within 5 working days after receiving complete dossiers, the dossier-receiving agencies shall have to examine the validity of the dossiers and notify them in writing to the establishments if the dossiers are invalid; If past 60 days from the date of receipt of the notice of invalid dossiers, if the establishments fail to reply or supplement and complete the dossiers as requested, the receiving agencies shall cancel the dossiers.

b) Basic evaluation:

– After the results of examination of valid dossiers, the competent bodies shall have to evaluate the establishments within 10 working days.

– Team appraisal team:

+ The appraisal team shall be established by the competent agency which issued the certificate or the agency authorized to appraise it.

+ The base evaluation delegation consists of from 5 to 9 persons (for small food producers and traders from 3 to 5 persons).

– The contents of the evaluation of the establishment: Collating information and verifying the legality of dossiers of application for certificates of food hygiene and safety with the original dossiers retained at the establishments according to regulations; Verification of food safety conditions at the establishment.

c) Issuance of Certificate:

– In cases where establishments meet all food safety conditions, the dossier-receiving agencies shall grant certificates.

– In cases where the establishments fail to fully satisfy the food safety conditions and have to wait for completion, they must clearly state the contents and finish time, but not exceed 60 days. The appraisal team shall organize the re-appraisal when the establishments issue a written certification that they have fully satisfied the requirements on food safety conditions of the previous appraisal team;

– In cases where establishments fail to meet food safety requirements, the dossier-receiving agencies shall notify in writing the local management agencies thereof for inspection and request the establishments not to operate until be issued a certificate. Establishments shall submit their dossiers for consideration and issue of certificates according to regulations.
